Position Overview
We are looking for a dedicated Physical Therapist to become part of a collaborative outpatient rehabilitation team. This role focuses on helping patients regain mobility, reduce discomfort, and improve overall physical function through personalized treatment programs. The ideal candidate is passionate about patient-centered care, values teamwork, and is committed to achieving excellent clinical outcomes.

Key Responsibilities
Patient Assessment & Treatment
Conduct comprehensive evaluations to assess patients' physical limitations, mobility, and functional abilities.
Develop individualized treatment plans based on clinical findings and patient-specific goals.
Deliver evidence-based physical therapy interventions to promote recovery, reduce pain, and improve overall function.
Reassess patient progress regularly and adjust treatment plans to maximize outcomes.

Patient Education
Educate patients and family members on prescribed exercises, recovery expectations, injury prevention, and techniques to maintain long-term physical health.
Encourage active participation in rehabilitation by providing guidance on home exercise programs and self-management strategies.

Documentation & Clinical Records
Maintain complete, accurate, and timely documentation of evaluations, treatment sessions, progress notes, and discharge summaries.

Care Coordination
Collaborate with physicians, referring providers, and other members of the healthcare team to coordinate comprehensive patient care.
Communicate patient progress and recommendations to appropriate healthcare professionals to support continuity of care.

Qualifications
Education
Doctor of Physical Therapy (DPT) degree from an accredited program. Licensure
Current and active Florida Physical Therapist license.

Skills & Experience
Strong clinical assessment and treatment planning abilities.
Commitment to delivering patient-centered, evidence-based care.
Ability to work effectively both independently and as part of a multidisciplinary team.
