This role focuses on improving mobility, strength, flexibility, function, and overall quality of life while promoting patient safety and independence.ResponsibilitiesEvaluate patients and develop individualized treatment plans based on physician orders and clinical findingsCollaborate with physicians and the care team to establish treatment goals appropriate for the patient's home environmentAssess strength, mobility, gait, range of motion, balance, pain levels, and functional limitationsRecommend and train patients, family members, and caregivers on the proper use of assistive devices and adaptive equipmentImplement therapeutic interventions designed to improve function, reduce pain, and enhance independenceIdentify fall risks and recommend home safety modifications and preventive strategiesMonitor patient progress and modify treatment plans as neededCommunicate changes in patient condition and treatment response to the physician and Director of NursingEducate family members and caregivers on exercises, therapy techniques, and ongoing care needsComplete accurate clinical documentation, progress reports, and discharge summaries in a timely mannerTreat clients, families, caregivers, and team members with professionalism, courtesy, and respectQualificationsMaster 's Degree or Doctor of Physical Therapy (DPT) from an accredited physical therapy programCurrent Physical Therapist license in the state where services are providedSuccessful completion of the National Physical Therapy Examination (NPTE) and any state-required licensing examinationsMinimum of two (2) years of experience as a Physical TherapistHome health or community-based experience preferredStrong clinical assessment, communication, and documentation skillsCommitment to maintaining HIPAA compliance and patient confidentialityJoin Our TeamIf you're passionate about helping patients regain independence and want the flexibility to create a schedule that works for your life, we'd love to hear from you.
