Facility Resurrection Medical Center Location 7435 W Talcott Ave, Chicago, IL 60631 Department Physical Rehab Schedule PRN (as needed) – Rational Weekend Employment Status Per Diem Shift Days Pay Transparency Flat rate of $43.09 per hour. Exact starting compensation will be determined at the time of hiring, taking into account skill set, experience, education, credentials, and licensure. Overview The Nurse Navigator will coordinate services and identified patients through the complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and the community. This role involves coordinating care in a timely and efficient manner, identifying and eliminating barriers to care, and serving as a consistent care coordinator throughout the continuum of care. The Nurse Navigator assesses the physical, psychological, and social needs of patients to enhance outcomes, increase satisfaction, and reduce costs of care, while also handling community education, screening, monitoring outreach, and promoting survivorship.
