Summary:

• The Patient Safety and Quality Improvement Specialist is responsible for facilitation and oversight of safety and quality performance improvement initiatives throughout Doylestown Hospital. This individual will be tasked to improve quality of patient care through process reengineering, safety strategy optimization, utilization of quality improvement tools and comprehensive data management and analysis.

Responsibilities:

• Manages and responds to events by conducting thorough event investigations, completion of Root Cause or Apparent Cause Analyses as appropriate, partnering with internal experts to identify and implement system improvements, as well as maintains event tracking for trend evaluation. Reviews event reports as entered in the Meditech system, assuring appropriate follow up and classification according to the Pennsylvania Patient Safety Authority. Maintains accurate record of these efforts in Meditech. Demonstrates strong interviewing skills to effectively gather event details from associates. Navigates the clinical chart for accurate timeline generation, paying attention to detail in generation of those timelines. Facilitates teams toward actions that prevent recurrence of events, utilizing established improvement tools.
• Collaborates and guides multidisciplinary teams through disciplined improvement approach to achieve desired results. Partners with organizational leaders regarding identification of process and outcomes measures, establishment of data collection tools and strategies, collection of baseline data and providing analyses to establish areas of improvement focus. Utilizes rapid cycle turnaround methods to encourage project momentum and evaluate improvement strategies. Provides guidance for improvement teams and departmental QAPI projects through coaching in all phases of project management for the achievement of their goals. Applies FMEA methodology for high risk processes with demonstrated avoidance of failures and harm within those processes.
• Serves as a professional resource and organizational support for Joint Commission accreditation, Pennsylvania Department of Health regulations and Center for Medicare and Medicaid conditions of participation as applicable to the hospital environment. Participates in survey and licensure activities in collaboration with other members of the Quality Initiatives staff. Provides leadership in implementation of plans of correction following onsite reviews, accreditation and certification surveys as well as citations requiring action plan development and execution. Participates and/or leads activities to assure evidence of standards compliance.
• Strategically develops, evaluates, and executes the organizational quality, patient safety and performance improvement program in partnership with the Director of Quality Initiatives Reviews and evaluates information from outside safety organizations for improvement and growth opportunities within Doylestown Hospital. Elicits and encourages reporting of safety events, both those that reach the patient and those with the potential to negatively impact patient care. Develops safety and improvement messaging for associates as ongoing strategy, utilizing SBAR format as appropriate
