As an integral member of our rehabilitation team, your duties will include assessing patients either in an inpatient or outpatient setting and crafting tailored treatment plans. Coordinating with nurses, physicians, and other rehab professionals ensures that patient outcomes are optimized. You'll not only implement physical therapy interventions aimed at enhancing mobility, strength, balance, and function but will also take the time to educate patients and their families about therapeutic goals and discharge planning, reinforcing the importance of home exercises. Your expertise will shine as you document evaluations and treatment progress in accordance with regulatory guidelines. Active participation in care team rounds and interdisciplinary conferences will ensure the delivery of well-coordinated care. To thrive in this role, you will need a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution, along with state licensure as a PT. While CPR certification is preferred, specialized certifications such as those for neurological or geriatric care are a valuable asset, but not mandatory. Previous experience handling patients with complex medical and mobility challenges is strongly preferred, along with exceptional communication abilities and teamwork skills. Navigating digital charting and clinical documentation systems comfortably will be essential in your day-to-day responsibilities.
