Job Summary:

Position Title: LPN LTC
Location: Green Bay, WI
Duration: 13 weeks
Work Schedule: 8/16 hrs. shift
Pay Rate: 45-50$/hr.
Responsibilities

• Provide direct nursing care to residents in a Long-Term Care (LTC) setting.
• Administer medications and treatments as prescribed by physicians.
• Assess, monitor, and document residents' conditions, reporting any changes promptly.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Maintain accurate and timely clinical documentation.
• Ensure compliance with infection control, patient safety, and facility policies.
• Educate residents and families on medications, treatments, and health management.
• Deliver compassionate, resident-centered care while promoting quality outcomes.
