MDS Coordinator

MDS Coordinator South Hampton Rehabilitation and Health Care Center No on-call required. Hybrid remote position (scheduled remote days). Compensated with a $10,000 sign‑on bonus. Job Summary Responsible for timely and accurate completion of the RAI and care management processes from admission to discharge, including Minimum Data Set, discharge and admission tracking. Works under the Director of Nursing and VP of Clinical Reimbursement and may coordinate information system operations and education for the clinical department. Qualifications Fire and passion for working with seniors. Flexible, fun, and energetic personality. Licensed as an LPN or RN in the state of practice. Prior MDS experience preferred. Minimum of one year experience in a LTC/SNF/AL/MC setting preferred. Excellent knowledge of Case‑Mix, Medicare PPS process, and Medicaid reimbursement preferred. Thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set. Knowledge of the care planning process. Professional image in appearance and behavior. Excellent written and oral communication skills.
