What You'll Do

• Perform comprehensive physical therapy evaluations and reassessments
• Develop individualized treatment plans based on patient goals and physician orders
• Provide evidence-based therapeutic interventions to improve mobility, strength, balance, endurance, and function
• Complete Start of Care assessments and ongoing therapy visits
• Assess patients' home environments and recommend equipment or safety modifications
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Collaborate closely with physicians, nurses, occupational therapists, speech therapists, medical social workers, and home health aides
• Complete timely and accurate electronic documentation
• Help patients safely recover while reducing hospitalizations and maximizing independence

Qualifications

Required

• Graduate of an accredited Physical Therapy program
• Current Arizona Physical Therapist license
• Current CPR Certification
• Valid Arizona driver's license
• Reliable transportation and current auto insurance

• Strong communication and organizational skills
