We are seeking a dedicated and skilled Pharmacist to join our healthcare team. This is a full-time position with varied shifts consisting of 4 ten-hour workdays. Weekend and holiday work may be required as needed.

Job Responsibilities

• Review medication orders, evaluating their appropriateness and completeness based on patient data such as age, height, weight, diagnosis, laboratory values, and other relevant information.
• Communicate with prescribers to suggest improvements and resolve issues with medication orders.
• Document resolutions and clinical interventions accurately and follow guidelines set by the Pharmacy and Therapeutics Committee regarding formulary management.
• Utilize clinical pharmacy skills to assess patient data and formulate recommendations to enhance patient outcomes, ensuring safety and quality in drug therapy.
• Participate in decentralized pharmacist activities as appropriate.
• Review, verify, and enter medication orders into the management information system in a timely and accurate manner.

Minimum Requirements

• At least 1 year of experience in an acute care setting.
• Current pharmacy licensure.
