Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home Health Experience

Assignment Details

• Shift Days: M-F
• Coverage area includes Deerfield south to the Ohio border and overflow coverage in the northern Downriver area.
• Expected travel may extend within an hour radius based on referral needs.
• EMR: Home Care Home Base
• On-call requirement: None
• Productivity expectation: 30 points per week
