Be a part of the Insight Chicago team that provides patient care second to none!/ph3Critical Care Clinical Specialist/h3pThe critical care clinical specialist provides quality pharmaceutical care defined as being responsible for assuring the provision of safe, efficacious and cost-effective drug therapy for all patients. The pharmacist will utilize integrate and routine communication with all health care professionals and the patient concerning medical care and quality of life with the emphasis placed on the monitoring and documentation of specific pharmacotherapy outcomes. The pharmacist provides assistance and in-depth drug information to medical personnel; performs operational/clinical functions or projects as assigned; meets qualifications to provide care for patients in the specific age range on their assigned clinical area. The pharmacist performs duties to provide the highest level of patient care and maximize patient satisfaction. These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Hospital and Medical Center. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, and therapeutic duplication and drug interactions. Discuss medication order clarifications with the prescriber, documents all changes in patient and pharmacy records. Assumes responsibility for the delivery of pharmaceutical care to patients in the critical care areas (CCU, ICU). Provides timely, efficient, cost-effective and appropriate care./liliMaintain and update patient profiles with demographics, allergies, and current medications on a daily basis. Promote the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary./liliIs competent in the range of treatment needed by the following patient as appropriate to the ages (adult, pediatric, geriatric, adolescent, and psychiatric)./liliInspects pharmacy records to monitor drug therapy for appropriateness and safety consistent with a high level of patient care. Prevent detect, monitor, document, and report adverse drug reactions and medication errors./liliPerforms pharmacokinetics, IV to PO conversions, therapeutic interchanges, and other clinical pharmacy functions accurately and in a timely manner./liliPromotes safe, efficacious, and cost-effective medication therapy by participating in the provision and documentation of clinical pharmacy services and execution of medication utilization initiatives (e.g.; IV to PO, renal dosing). Responds to medication related inquiries from other health care professionals and patients. Document clinical interventions and cost avoidance as part of the departmental documentation program./liliServes as a consultant to hospital Pharmacy Therapeutics committee and other hospital sponsored clinical and performance improvement committees./liliIs active in clinical rounding, public speaking and presenting technical and financial data using PowerPoint presentation skills or similar format as assigned. Contribute to newsletter articles, in-services and educational material for pharmacy staff and other healthcare professionals to promote pharmacy as a respected source of drug therapy information and disease management resource./liliParticipate in the planning and implementation of system-wide pharmaceutical care programs to optimize drug therapy outcomes. Complete one clinical project per fiscal year. Project to be determined in collaboration with pharmacy clinical team./liliEnsures all patient care provided meets or exceeds all regulations, statutes, and HFAP standards./liliReports daily to Clinical Pharmacy Manager on overall operations, projects assigned, and expectations of overall role./liliResponds to medical emergencies as appropriate./liliCounsels patients on discharge medications with ICU and Behavior health floor education as necessary) and coordinate meds to beds and interventions with Outpatient Pharmacy when needed./liliRotating shifts AMPM./li/ulh3Job Specifications/h3pRequired knowledge, skills and abilities:/pulliPharm.D. or equivalent from an ACPE- accredited School of Pharmacy and a current pharmacist license in good standing with appropriate State Board of Pharmacy. Critical Care PGY1,2 residency recommended or at least 3 years appropriate clinical experience is recommended. Board Certification Recommended. Experience using computer applications including word processing, spreadsheets, and pharmacy information systems./li/ululliThorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem solving ability and analytical thinking preferred./li/ululliAbility to work independently and handle stress appropriately. Ability to work cooperatively with other members of Insight Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule.
