The Role
- Provides expertise in the assessment, plan development, coordination, and monitoring of care services to ensure the highest quality of care
- cts as a guide for our Clients by navigating the complexities of the health care system
- ssists in making decisions from a neutral point of view as family members sometimes have to make difficult decisions that are deeply personal
- CCM's enhance the quality of life in our Clients by organizing care plans and advocating Client's needs by working closely with social workers, attorneys, healthcare providers, and other providers in collaborative efforts to support some of the frailest of our community.
- Evaluates in-home care needs
- ddresses emotional needs and concerns
- Coordinates medical services
- Monitors and advocates for medical needs
- Enhances social and quality of life activities
- Establishes short-term and long-term plans
- Discusses difficult topics and complex issues
