What You ll Do

• Provide comprehensive outpatient primary care to patients across the lifespan
• Conduct physical examinations, interpret diagnostic testing and develop individualized treatment plans
• Manage chronic conditions and provide patient education and preventive care
• Maintain and manage an established patient panel
• Collaborate with behavioral health, wellness and other clinical and non-clinical team members
• Coordinate specialty referrals and inpatient care when appropriate
• Complete necessary patient documentation and forms, including DME, DMV, IHSS, EDD/disability and school-related forms
• Manage Epic In-Basket responsibilities
• Participate in quality improvement initiatives, peer review and clinical audits
• Participate in a rotating after-hours physician call schedule
• Occasionally participate in community health events and health fairs

Qualifications

• MD or DO from an accredited medical institution
• Board Certified in Family Medicine, Internal Medicine or a related primary care specialty
• Active, unrestricted California medical license
• Active and unrestricted DEA registration
• Minimum 2 years of primary care experience
• Current BLS certification
• Comfortable managing a full-time patient panel of approximately 1,500 patients
• Strong commitment to serving underserved and multicultural communities
• Collaborative approach to multidisciplinary, team-based care
• Strong written and verbal communication skills
• English proficiency required; additional language proficiency is a plus
