Position Requirements: Licensed Social Worker in California (LCSW or MSW preferred). CPR Certification. Recent physical exam within the past year. Clear TB Test. Role Responsibilities: Evaluate and support patients in their homes, focusing on emotional well-being, family dynamics, discharge planning, and resource coordination. Provide support to patients and their families in navigating healthcare systems and connecting with community resources. Document progress with easy-to-use laptop software. Manage a caseload of 1 to 7 patients per week, ensuring each patient receives compassionate and personalized care.
