Primary Care Nurse Practitioner or Physician Assistant

Overview Hartford Healthcare seeks a Primary Care Nurse Practitioner or Physician Assistant to deliver comprehensive, patient-centered primary care within our Healthcare & Medical Services team. In this role, you will conduct assessments, diagnose and manage acute and chronic conditions, prescribe medications, and coordinate care across our network.
