The Family Medicine Clinician provides comprehensive, culturally-appropriate, patient-centered primary care services to eligible patients of the St. Croix Tribal Health Division in accordance with professional standards, organizational policies, and applicable Indian Health Service (IHS) guidelines.

Job Description

Position Summary:

The Family Medicine Clinician provides comprehensive, culturally-appropriate, patient-centered primary care services to eligible patients of the St. Croix Tribal Health Division in accordance with professional standards, organizational policies, and applicable Indian Health Service (IHS) guidelines.

Essential Functions:

• Evaluate, diagnose, and treat acute and chronic conditions.
• Conduct examinations and order and interpret diagnostic tests and screenings.
• Provide preventive care, health education, and counseling regarding healthy lifestyle habits across the lifespan.
• Perform office procedures and other services consistent with training, competency, and privileges.
• Coordinate referrals and care with specialists and other members of the healthcare team.
• Prescribe and manage medications
• Provide substance use disorder treatment, including medications for opioid use disorder, when appropriately trained and privileged.

Family Medicine Clinician Job Duties:

• Provide medical services to eligible patients at St. Croix Tribal Health Clinic locations, satellite sites, and community events as needed.
• Collaborate with the Medical Director and other healthcare professionals regarding patient care and clinical matters.
• Participate in clinical quality improvement, patient safety, and outcomes review as requested.
• Safeguard and care for all medical equipment and recommend needed medical supplies.
• Maintain required licenses, certifications, credentials, and professional qualifications.
• Complete documentation necessary for patient care, coding, billing, and reimbursement.
• Utilize the clinic EHR for patient care, documentation, ordering, referrals, and other required clinical functions. Complete charting within forty-eight (48) hours of service or as otherwise defined by organizational policy.
• The Family Medicine Clinician will accurately complete and process referral forms and such other forms, reports and documents in accordance with St. Croix rules and regulations.
• Maintain patient confidentiality and comply with applicable professional, regulatory, and organizational requirements.
• Participate in interdisciplinary teams, staff meetings, clinical education, and other activities reasonably necessary to support effective delivery of patient care.

Directly Reporting to: The Family Medicine Clinician works under the leadership of the Medical Director and within the clinician's applicable scope of practice, credentialing, and privileges which may include collaborative practice or supervision. Administrative supervision is provided by the Medical Director in accordance with the Health Division organizational structure.

Matrix of INDIRECT Reporting: Health Division Head

Scope and Complexity:

Interacts with all levels of management and employees; often trusted with maintaining confidentiality and relaying highly sensitive messages, and appropriate application of business operations and practices.

Physical Demands/Environment Factors:

• Requires extensive sitting with periodic standing and walking.
• May be required to lift up to 50 pounds.
• Requires significant use of computer and general office equipment.
• Personal smart phone required for approval of controlled substances and strongly encouraged for use of mobile EMR application.
• Needs adequate visual acuity, ability to grasp and handle objects.
• Need ability to communicate effectively through reading, writing, and speaking in person or on the telephone.
• May require off-site travel.

Minimum Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Wisconsin licensure as either:
• An Advanced Practice Registered Nurse and appropriate national board certification; or
• A Physician Assistant (PA)
• Current DEA certification/eligibility
• Competence in the use of Microsoft Office computer applications
• Must have the ability to prescribe medications.
• Must have the ability for claims to be submitted to the Center for Medicare and Medicaid Services (CMS)
• Sound understanding of all federal and state regulations including HIPAA and OSHA
• Valid driver's license and ability to travel between locations or to occasional community events.

Preferred Qualifications:

• Experience in a primary care setting as a family medicine clinician.
• Practice experience providing services to American Indian/Alaska Native, rural, or underserved populations.
• Competence in the use of EHR/MDR systems.
