About This Role As a Nursing Admission Specialist, you will initiate and/or resume home care for patients of all ages in the east region, providing comprehensive assessments, care plans, and ongoing support. Highlights Pay: $73,840 - $115,648 annually Location: Independence, OH Facility: Cleveland Clinic Main Campus Schedule: Full-time, Day shift What You'll Do Conduct and submit a complete and accurate Comprehensive Assessment in accordance with OASIS reporting requirements. Identify immediate care and support needs and obtain orders to address psychosocial and treatment priorities. Make appropriate admission decisions based on patient eligibility, acuity, and safety. Obtain consent for care. Demonstrate knowledge of coverage criteria based on payer source.
