Shifts Available: Friday - Sunday : 8am-4pm . Note additional hours on Saturday and Sunday may be available. We are only considering applicants who can work this schedule

Description and Responsibilities:

The Direct Care Professional provides person-centered support to adults with intellectual and physical disabilities. The position promotes each individual’s health, safety, independence, dignity, and progress toward the goals identified in the Individual Service Plan (ISP) and My Plan. Responsibilities include:

·         Provide direct care and support in accordance with each individual’s Individual Service Plan (ISP) and required documentation.

·         Assist individuals in working toward or achieving the goals identified in their My Plan.

·         Administer medications and accurately complete the Medication Administration Record (MAR) in accordance with applicable requirements and agency procedures.

·         Facilitate skill acquisition in the following areas: Personal Hygiene, Meal Planning and Preparation, and Community Skills

·         Maintain a safe environment and educate individuals on Fire and Tornado safety

·         Be flexible, patient, energetic, creative

·         Relate to the individual in a way that fosters a positive atmosphere

·         Transport individuals on an as needed basis.

·         Attend any medical, eye, dental, and any necessary appointments per doctor’s orders. 

·         Attend and actively participate in monthly staff meetings.

·         Comply with agency policies and procedures.

·         Other duties as assigned.

QUALIFICATIONS:

·         High School Diploma/GED

·         Criminal & Investigative Background Check with no disqualifying offenses.

·         Must be able to pass DODD/Medicaid Background Investigation Requirements

·         Must have reliable transportation with automobile insurance that complies with state law.

·         Must be certified in CPR & First Aid

·         Must be certified in DODD’s Medication Administration Training (At least level 1)

·         Experience working with individuals with disabilities (At Least 1-2 Years of Experience).

PAY: $18.00 Per Hour

BENEFITS:

·         Sign-On Bonus - $1000.00 sign-on bonus. $500.00 will be paid on the first check and the remaining $500 will be paid on the next pay cycle after 90 days of employment. Note sign-on bonus is subject to tax withholding and terms.

·         Two Sick Days and one Personal Day per Year - Available after One-Year Anniversary. Terms and conditions apply

·         Four Hour Minimum - You will automatically be paid for four-hours of service if you deliver services less than four-hours in a day. Terms and Conditions apply.

·         Paid Time Off (PTO) - At your one-year anniversary, you are eligible for up to 40 hours of PTO. Ask for details!

·         Performance Based Awards - At CCMS, we award our staff for exceptional performance. At every staff meeting, we select an employee of the month and they receive a bonus.
