Lehigh Valley Health Network - 2024 Lehigh Street - Responsibilities: Coordinate bed availability and patient transfers to optimize placement; Collaborate with departments for inpatient transfers to meet admission criteria and obtain admit numbers; Document required information in patient records and transcribe transfer interactions; Answer transfer center calls and coordinate emergent information and referrals; Triage and prioritize patient transfers to specialty care; identify, resolve, and escalate major issues impacting transfers
