Geriatric Care Case Manager

Northern Kentucky Area Development District in Florence seeks a Case Manager to provide assessment and ongoing case management for Participant Directed Services (PDS) Waiver participants. The role includes eligibility evaluation, care planning, and monitoring services to meet participant needs. The position requires home visits, collaboration with participants, caregivers, and providers, and accurate documentation in the case management system.
