Role Description
We are looking for a nurse to provide 1:1 skilled nursing home care with a patient located in Macomb, MI. The patient receives 24/7 care through 12 hour nursing shifts.
Open Availability: Every other weekend, 7 AM - 7 PM
Qualifications
Active, unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) license in the state of Michigan and current BLS/CPR certification.
Demonstrated clinical skills in assessment, medication administration, blood pressure management, and chronic disease management in home health or related settings.
Strong documentation and organization abilities, including accurate charting, care planning, and adherence to regulatory requirements.
Excellent communication and interpersonal skills to collaborate with patients, families, physicians, and interdisciplinary team members.
Ability to work independently in the field, manage a caseload, and adapt to changing patient needs and schedules.
Proficiency with basic computer systems and electronic health record (EHR) platforms.
Prior home health, hospice, or community nursing experience preferred
A patient meet and greet, followed by a training shift are required.
