POSITION SUMMARY

Under the Supervision of the Operations Director, the Qualified Intellectual Disabilities Professional (QIDP) develops, implements, and oversees individualized Comprehensive Functional Assessment plans and active treatment for individuals with intellectual and developmental disabilities, ensuring compliance with regulations and providing support to staff. Key responsibilities include writing service plans and goals, documenting client progress, person-centered planning, coordinating care with an interdisciplinary team, team leadership, regulatory compliance, overseeing service provisions, and coordinating program operations in residential, day habilitation, or community integration settings. The QIDP’s central goal is to ensure that clients receive active treatment to enhance their independence, personal growth, and quality of life in the least restrictive environment possible. The QIDP also provides guidance and support to families and other professionals.

DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS

Coordinates services, integrates and monitors active treatment, and manages the overall programming for individuals with intellectual and developmental disabilities. Creates and implements Comprehensive Functional Assessment plans and activity plans, sets learning goals, and develops participant-specific training based on individual needs and diagnoses and ensuring their preferences are included. Leads and facilitates the Interdisciplinary Team (IDT) meetings made up of specialists including therapists, direct care staff, and other clinicians; coordinates and integrates the services provided by the team. Documents client progress, completes and maintains timely written documentation, monthly notes, and prepares program reports for management, ensuring all actions comply with state and federal regulations, including Medicaid, Department of Health (DOH), and OPWDD. Oversees service provisions provided by DSPS and other staff as outlined in the individualized service and care plans to ensure quality. Provides training and guidance to staff on implementing client goals, behavior support plans, and proper procedures. In partnership with and under the guidance of the Operations Director, oversees the operations of the residence and guides staff in the absence of the Residential Manager. Serves as a liaison between clients, their families, and advocates, keeping them informed of the client's progress and addressing any concerns. Provides feedback to the Residential Manager regarding DSP performance and compliance with individualized staff action plans, behavior plans, and protective oversight plans to be included in performance appraisal and corrective actions as needed. Ensures the client's rights are protected and advocates for needed services. Monitors program implementation, audits files, and conducts regular observations to ensure high-quality care is being provided. Responds to and manage crisis situations according to agency policies and procedures. Ensures that all required documentation (paper and electronic) is completed/processed appropriately, in a timely manner, and meets quality and compliance thresholds. Performs other duties as assigned. Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk. Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours. Meets all job requirements and stays in compliance. Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations. QUALIFICATIONS

Education: Bachelor’s degree in human services, psychology, sociology, social work, special education, or another related field required. Experience: Minimum of two (2) years of experience supporting individuals with intellectual/developmental disabilities and/or enhanced medical needs required. Minimum of one (1) year performing auditing and/or compliance tasks or similar functions required. Specialized Training, Licenses, Certifications: SCIP and CPR/FA required (provided upon hire if needed) AMAP and G‑Tube certification (must be completed within 120 days of hire and maintained thereafter) Knowledge, Skills, Competencies, and Abilities: Knowledge of chronic conditions common in IDD populations (e.g., Diabetes, mobility impairments, Skin Integrity issues) Ability to consistently exercise discretion and judgment relating to care plans and individual services Ability to analyze and interpret information to determine effective ways to implement plans and correct deficient areas. Knowledge of health and safety practices, relevant laws, regulations, and quality assurance measures in disability services. Understanding of residential care systems, OPWDD regulations, and person-centered planning. Strong skills in care coordination, advocacy, critical thinking, and collaboration. Proficiency with electronic documentation systems and data entry. Proficient communication, organizational, and interpersonal skills. Computer literacy and ability to learn and use multiple applications, including Medisked and Microsoft Office Suite. Valid Driver’s License and satisfy criteria to be an agency driver required. Ability to work a flexible schedule based on the needs of the programs supported. Ability to be on‑call and address emergency situations as they arise.

WORK ENVIRONMENT

This role is performed in‑person with travel required to various locations including residential ICF programs, day programs, community programs, hospitals, doctor’s offices, FREE locations, and other community locations. Work assignment location varies based on need.

PHYSICAL REQUIREMENTS

Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours). Employee is required to effectively communicate both verbally and in writing. Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets. Employee is required to reach with hands and arms. Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed. Employee is required to be able to lift 50 pounds at a time.
