The therapist works collaboratively with other patient team personnel in maintaining standards of professional physical therapy practice. Therapist uses independent judgment, assumes responsibility and accountability for the delivery of physical therapy services provided by Physical Therapist Assistant. Responsible to orient staff and assume the duties and responsibilities.The incumbent will carry out her/his duties by adhering to the highest standards of ethical and moral conduct, acts in the best interest of Three Crosses Regional Hospital, and fully supports the mission, vision, and values of Three Crosses Regional Hospital. The incumbent will have knowledge of health care systems, and have demonstrated ability to interact, relate to, work with, and support the activities of a diverse workforce.Primary ResponsibilitiesProvides high quality patient care and assessment in accordance with established practice standards and policies in a safe environment.Reviews patients' medical history, referrals and physician notes from the patient charts or other medical records.Establishes a plan of care to improve patients' functions and movement.Provides treatment in accordance with physician's orders and established plan of care.Evaluates effectiveness of treatment plan and revises plan of care as indicated.Administers treatment programs for patients not limited to therapeutic exercises and activities, application of physical modalities i.e. heat, cold and light.Evaluate and record patient's progress, modify plan of care and discuss with physical therapy assistants an established plan of care and treatment procedures.Educate patients and their families on recovery process and train families/caregivers on how to perform patient care.Assess patient health by interviewing patients, performing physical examinations, and obtaining, updating and studying therapy histories.Documents patient care services promptly on medical charts.Performs therapeutic procedures by administering exercises and instructing, encouraging, and assisting patients in performing physical activities, ambulatory functional activities, and activities of daily living.Determine appropriate assistive devices such as crutches, canes and prostheses.Instructs and counsels patients by describing therapeutic regimens, giving normal growth and development information, and promoting wellness and health maintenance.Maintains a safe and clean working environment by complying with procedures, rules and regulations and adhering to infection control policies and protocols.Maintain production objectives by noting current trends and monitoring actual physical therapy services rendered to production objectives.Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient populations served.Plans and make appropriate discharge plans.Assesses the risk for safety and implements appropriate precautions.Utilizes appropriate leadership skills in delegating, organizing, and educating coworkers and staff.Participates and contributes to quality improvement process.Makes effective use of patient schedule and time by achieving efficiency standards such as, visits per day and evaluations.Identifies assigned rotation/floor needs.
