Care Coordinator Case Manager - Bilingual English Spanish Preferred

Position Overview The Care Coordinator is a key member of MEMBERS ONLY SIGN UP NOW . ’s (TCC) care coordination team, providing direct support to LGBTQIA+ individuals navigating mental health, housing, healthcare, and basic needs systems. This role is grounded in relationship building, advocacy, and practical problem solving - ensuring that each client is met with dignity, affirmation, and a clear path forward. The Care Coordinator works closely with clients, staff, and community partners to deliver responsive, client-centered, and trauma‑informed care. This position plays a critical role in advancing TCC’s mission to ensure LGBTQIA+ individuals have access to the resources and support they need to thrive. Areas of Responsibility Direct Client Support and Case Management Provide individualized care coordination, including intake, assessment, goal setting, and ongoing follow‑up Develop and implement client‑centered service plans that reflect each individual’s needs, priorities, and strengths Support clients in accessing mental health care, housing resources, healthcare, and basic needs services Maintain consistent, relationship‑based communication to foster trust, engagement, and continuity of care Respond to urgent needs with a calm, grounded, and solutions‑oriented approach Resource Navigation and Advocacy Connect clients to local and regional service providers, ensuring warm handoffs whenever possible Advocate for equitable, affirming access across systems, particularly for LGBTQIA+ individuals facing systemic barriers. Maintain a strong and current understanding of community resources and eligibility requirements. Help clients navigate complex systems, including benefits, healthcare, and housing processes Documentation, Data, and Coordination Maintain accurate, timely, and compliant case documentation Track client outcomes, referrals, and service utilization to support program evaluation and reporting Collaborate with internal staff to ensure coordinated, holistic care across programs Contribute to data‑informed decision‑making and continuous improvement efforts Community Engagement and Organizational Support Support outreach efforts to increase awareness and accessibility of care coordination services, including tabling and community events Participate in and contribute to TCC’s annual Pride in the Park, supporting planning and/or day‑of execution as needed Attend occasional evening and weekend programs or events, based on availability; all additional hours are eligible for comp time Represent TCC in community spaces in a way that reflects its mission and values Who You Are Client Centered and Trauma Informed You approach this work with a deep respect for the dignity, autonomy, and lived experiences of the people you serve.
