We're seeking a dedicated Physical Therapist to join our outstanding rehabilitation team on a Part-Time or Per Diem basis. What You'll Do Evaluate residents and develop individualized treatment plans Provide skilled physical therapy services to post-acute and long-term care residents Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families Monitor patient progress and modify treatment plans as needed Maintain accurate and timely documentation Educate residents and caregivers on exercises, safety, mobility, and discharge recommendations Help residents achieve their highest level of independence and quality of life Qualifications Required: Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program Current California Physical Therapist license in good standing Strong clinical assessment and treatment skills Excellent communication and teamwork abilities Preferred: Experience in Skilled Nursing, Post-Acute, Rehabilitation, or Long-Term Care settings Knowledge of Medicare documentation and reimbursement guidelines Experience treating orthopedic, neurological, and medically complex patients Why Citrus Heights Post Acute?
