Clinician (Street Outreach and Engagement Program) Location: Integrity House - Newark Are you searching for a fulfilling place to develop your career and make an impact? Integrity House operates 16 licensed treatment programs across New Jersey as well as community outreach and prevention programs, and serves over 3,000 individuals every year. Duties and Responsibilities of a Clinician (Street Outreach and Engagement Program) Conduct behavioral health assessments using evidence-based assessment tools to determine appropriate behavioral health interventions, levels of care, and referrals. Assess participants for behavioral health crises, including suicide risk, substance-related intoxication, withdrawal, and overdose, and determine the need for emergency psychiatric evaluation, medical intervention, withdrawal management, or other crisis services. Provide crisis intervention, de-escalation, motivational interviewing, and other evidence-informed interventions to support participant stabilization and engagement. Collaborate with Outreach Specialists to assess participant needs, determine appropriate interventions, and support service planning. Develop individualized service recommendations based on participant needs strengths, and goals. Facilitate referrals and warm handoffs to mental health services, substance use treatment, crisis, and other clinically indicated services. Collaborate with Outreach Specialists, Mobile Crisis, hospitals, behavioral health providers, and other community partners to coordinate participant care and promote continuity of services. Advocate for participants during psychiatric evaluations, emergency department visits, hospitalizations, and discharge planning to promote continuity of care. Conduct clinical follow-up following crisis intervention, hospitalization, or referral to support continuity of care and ongoing connection to services. Participate in multidisciplinary case conferences and coordinated care planning. Complete timely and accurate documentation in HMIS and other required systems while maintaining compliance with HIPAA and agency policies. Participate in clinical supervision, staff meetings, training, and quality improvement initiatives. Perform other duties as assigned by management. Required Education and Experience Master’s degree in Social Work or Counseling, or a closely related behavioral health field from an accredited institution. Minimum one (1) year of experience working with individuals experiencing homelessness, substance use disorders, mental health conditions, co-occurring disorders, or related social service needs. Required Licensing or Certifications New Jersey LSW, LCSW, LAC, LPC, or LCADC preferred. CPR certification within 30 days of hire. Valid New Jersey driver’s license with an acceptable driving record. Specialized Skills, Abilities and Knowledge Experience conducting behavioral health assessments, crisis intervention, and care coordination in community settings. Knowledge of ASAM Criteria and the behavioral health continuum of care. Understanding of trauma-informed care, Motivational Interviewing, and low-barrier engagement strategies. Ability to recognize and respond appropriately to behavioral health crises. Strong organizational, communication, documentation, and problem-solving skills. Ability to build trusting therapeutic relationships with individuals experiencing complex behavioral health and social needs. Work Schedule This position is available on a full-time or part-time basis. The Street Outreach and Engagement Program operates seven (7) days per week from 4:00 PM to 12:00 AM.
