We are seeking a Physical Therapist Assistant (PTA) to join the rehabilitation team at Autumn Lake at Madison. This full-time opportunity is ideal for a PTA who enjoys hands-on patient care and helping residents improve strength, mobility, balance, and overall independence.
What You'll Do

• Provide physical therapy treatments under the direction of a licensed Physical Therapist
• Assist residents with strengthening, balance, gait training, transfers, endurance, and mobility
• Carry out individualized treatment plans established by the supervising PT
• Monitor resident progress and communicate updates to the therapy team
• Educate residents and caregivers on exercises and safe mobility techniques
• Complete accurate and timely clinical documentation
• Collaborate with therapy, nursing, and interdisciplinary team members

Qualifications

• Active Connecticut Physical Therapist Assistant license required
• Graduate of an accredited Physical Therapist Assistant program
• Strong communication and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduate PTAs are welcome to apply
