Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. The clinical pharmacist will serve as the central coordinator within a multidisciplinary team, ensuring accurate sterile compounding, dosage adjustments, and providing essential education to patients and caregivers.

Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in sterile, controlled environments, adhering to state and federal regulations.
• Assess laboratory values, calculate pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatments.
• Deliver comprehensive counseling and education to patients and caregivers about specialty medications, infusion pump use, sterile techniques, and catheter care.
• Collaborate with prescribing physicians, home health nurses, and internal teams including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations for coordinated patient care and to minimize hospital readmissions.
• Ensure compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and others.
• Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for secure, timely home delivery.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• IVIg experience is a valuable asset.
• Strong knowledge of specialty therapy payer requirements, reimbursement procedures, and contract compliance.
• Current and active NY pharmacist license in good standing.
• Familiarity with CareTend system preferred.
