Position Information Location: Wayne, NJ Facility: CareOne at Wayne Salary Range: $40.00 – $63.00 per hour Practice Setting: Short‑Term Rehab and Long‑Term Care Essential Duties And Responsibilities Provide and direct resident rehabilitation care within accepted practice methods, meeting all center, state, federal, and accrediting standards. Evaluate resident needs and establish effective treatment plans for assigned residents. Manage and coordinate the supervision of treatment plans with all staff and other disciplines as indicated. Participate in the screening of all residents according to directed schedules and practices. Provide feedback to the designated supervisor on all clinical and rehabilitation issues. Participate in all center‑directed processes and services. Perform necessary clinical documentation to meet center, state, federal, and accrediting agency requirements. Maintain complete, accurate, and timely billing records to meet center, state, federal, and accrediting agency requirements. Participate in in‑service training programs as requested. May travel to other facilities to provide services on a limited basis. Position Requirements Master’s or Doctorate Degree in Physical Therapy. Current and active Physical Therapist license within the state of operations. Prefer experience or knowledge in older adult rehabilitation. Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings.
