Location: New York, NY 10065
Contract Duration: 13 Weeks
Schedule: Monday–Friday, 9:00 AM–5:00 PM
Lunch: 60-minute unpaid lunch break
Assignment Type: FTE Leave of Absence (LOA) Coverage

Weekly Pay: $2,600

Position Overview

We are seeking an experienced Licensed Master Social Worker (LMSW) to provide inpatient hospital social work services in a leading healthcare facility in New York City. The ideal candidate will have experience supporting patients and families with complex medical and psychosocial needs, with a strong background in oncology, discharge planning, goals-of-care discussions, and end-of-life planning .

The LMSW will work as part of a multidisciplinary healthcare team to assess patient needs, coordinate appropriate services, facilitate safe discharges, and provide emotional and psychosocial support to patients and their families.

Key Responsibilities

• Provide comprehensive inpatient hospital social work services to patients and their families.
• Conduct thorough psychosocial assessments and identify patient and family needs.
• Assist with discharge planning, care coordination, and transition-of-care services .
• Support patients and families experiencing complex medical and psychosocial situations.
• Participate in goals-of-care discussions in collaboration with the interdisciplinary care team.
• Provide support related to end-of-life planning and conversations .
• Assist patients and families with appropriate community resources, referrals, and support services.
• Collaborate closely with physicians, nurses, case managers, and other healthcare professionals.
• Provide counseling, emotional support, and crisis intervention as appropriate.
• Maintain accurate, timely, and comprehensive documentation in accordance with hospital policies.
• Advocate for patient needs while supporting continuity and coordination of care.

Required Qualifications

• Master’s Degree in Social Work (MSW) – Required.
• Active New York State LMSW License – Required.
• Previous inpatient hospital social work experience .
• Experience with discharge planning and care coordination .
• Previous oncology social work experience .
• Experience participating in goals-of-care discussions .
• Experience supporting end-of-life planning and conversations .
• Knowledge of psychosocial assessments and interventions .
• Strong written and verbal communication skills .
• Ability to work effectively within a multidisciplinary healthcare environment.
