Occupational Therapist - Home Health

Schedule: 8-hour days

Assignment Details: Guaranteed Hours: Facility may call off up to one shift per pay period.

Requirements:

• Minimum 1 year of skilled home health experience required
• OASIS experience required
• BCLS/BLS certification required
• OR and WA licenses required; pending license accepted

Roles & Responsibilities:

• Assess and evaluate patients' occupational performance and needs.
• Develop and implement individualized treatment plans to improve patients' ability to perform daily activities.
• Document patient progress and outcomes in accordance with regulatory and facility standards.
• Provide education and support to patients and their families regarding treatment and rehabilitation processes.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Compliance / Notes:

• References and compliance documents are required prior to start.
• Pre-start modules average ~16 hours (approval required for over 20 hours).
• Free parking available.
• Business casual attire preferred; scrubs allowed (no jeans).
