We’re hiring a Social Worker (MSW) – a compassionate and skilled professional who will provide emotional support, counseling, and resource coordination for patients and families during their hospice journey. What You’ll Be Doing Conduct psychosocial assessments, develop care plans, and document bereavement activities Design and distribute educational materials for families receiving bereavement services Develop programs on grief and loss for families and staff Evaluate the bereavement program quarterly and annually Provide community consultation and education on hospice services Maintain ongoing education through academic studies and workshops Attend in-services, staff meetings, and case conferences Report patient condition changes to the RN Case Manager Observe and address safety hazards in patient and organizational environments Assist the health team in understanding emotional factors related to terminal illness Assess suicide risk, mental capacity, and elder/dependent adult abuse Aid in advance directives, mortuary arrangements, and financial planning Provide information and referrals to community resources and legal entities Advise and counsel staff, patients, families, or agencies in emergency situations Qualifications Master’s degree in Social Work required Documented evidence of training in grief and loss counseling Valid driver’s license, good driving record, and reliable transportation Comfortable working in environments with pets Fortune Magazine Best Workplace in Aging Services Certified Great Place to Work Strong internal mobility Shift Schedule: Part Time, Monday - Thursday, 8:30 AM - 5:00 PM, must have an open availability for weekend on-call rotation Anticipated pay range: $35 USD - $39 USD Benefits 401(k) matching Health, Dental and Vision Insurance Flexible Spending Account (FSA) Paid time off Tuition reimbursement and professional development assistance Referral program Other amazing benefits such as Pet Insurance, Auto and Home Insurance and Discount Programs * Silverado does not accept candidates from outside recruiters or agencies for this role. * California Employees: For roles requiring driving, the driving requirement has been deemed essential to the performance of this role due to specific business necessity.
