At Valley ACT, our Crisis Care Nurses play a vital role in supporting individuals experiencing some of life's most challenging moments. Working as part of a multidisciplinary crisis team, you'll provide medical assessments, medication management, and compassionate care that helps clients stabilize, access treatment, and move toward recovery. Through clinical expertise, quick decision-making, and a person-centered approach, you'll make a meaningful difference when support is needed most.

Pay: Range starts at $71,760.00 annually (pay calculated based on years of licensed experience)

Schedule: Monday Friday | 8:00am 5:00pm (with alternating afterhours on-call)

Job Summary

The Crisis Care Nurse functions as a member of assigned Valley crisis teams and assists with determining the most appropriate treatment regimen to meet the identified needs of clients who need intensive treatment and services. The Nurse provides medication coordination, nursing assessments, and monitors client health care needs.

Essential Functions

• Provides medication management services including assessment of behavioral health status and medication administration and distribution as ordered by prescribers
• Provides care management services, personal rehabilitative services, and patient education in formal groups as indicated in the care plan
• Assists prescribers and clients with ongoing medication management in clinic and various community-based settings
• Provides client education and support regarding medications and medical concerns
• Triages medical needs based on severity and acuity
• Provides medical screening, treatment, referrals, and support
• Performs blood draws and completes necessary lab work
• Maintains all necessary documentation and clinical records
• Provide in-service training for program staff on common medical issues and appropriate treatment, intervention, and support strategies
• Bills for services in a timely manner
• Meets required client care and other organizational performance expectations
• Maintains after-hours and weekend on-call availability
• Follows agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions and discharge plans, and client care
• Collaborates with internal and external multidisciplinary teams regarding clinical aspects of treatment
• Adheres to ethical and legal standards set by licensing boards and state regulations.
• Meets direct care expectations
• Participates in ongoing professional development activities to meet licensure, certification, and/or other agency requirements
• Travels to work in assigned clinics

Requirements

Education

• Completion of a registered nurse training program from an accredited organization
• Graduation from ValleyAcademy (Post hire)

Licenses/Certificates

• RN license in state of practice
• CPR certification (Post hire)
• Valley de-escalation certification (Post hire)

Preferred Qualifications

• Previous ICU, ER, paramedic, or other crisis stabilization experience
• Previous behavioral health experience
