Triage Mental Health Counselor, Transfer Center Per Diem

Serve as the person in charge of triaging patient intake and transfer calls Prioritize behavioral services bed capacity Serve as liaison with system referral partners Provide supportive and informational communication with persons served Ensure optimal intake and bed placement of patients Manage referrals in the EMR and track and monitor referral status Answer phone calls and provide initial phone assessment and screening for bedded programs Ensure referral information is complete Use multiple computer systems to place clients and monitor program bed capacity Conduct ASAM screenings and provide referral information to individuals who do not meet admission requirements Maintain relationships and communication with hospital emergency rooms and referring partners Communicate programmatic updates and clinical procedural alerts Access the administrator on call and escalat referral information when needed Provide feedback to the Transfer Center Manager regarding shift performance Coordinate necessary information for methadone guest dosing and other medications before admission Maintain denial and acceptance in accordance with regulatory guidelines Perform other related duties as assigned Requirements Associate's degree (A.A.) or equivalent from a two-year college or technical school; or one or more years related experience and/or training; or equivalent combination of education and experience Bachelor degree preferred Ability to answer phones and hear and communicate fluently with English-speaking callers Ability to key into computer Ability to read documentation and computer entries Excellent communication skills Teamwork and ability to work cooperatively and effectively with others Ability to adapt to changes in the workplace Ability to pay attention to detail and multitask Must maintain professional status Ability to walk, stoop, kneel, crouch, or crawl Ability to occasionally lift and/or move up to 20 pounds Distance vision, peripheral vision, and depth perception Must be able to meet the employer's influenza vaccination requirement Core Competencies Demonstrates expertise in patient intake triage, referral management, and effective communication with healthcare partners.
