Schedule: Full Time Friday-Monday-4-day weekend baylor with Shared team on-call rotation

Territory: Manatee County with Shared team on-call rotation

What You’ll Do: 

• Provide and document skilled nursing carebased on individualized care plans and physician orders 

• Perform patient assessmentsand create personalized care strategies that support recovery and independence 

• Educate patients and familieson specific medications, symptom management and lifestyle changes to promote better patient outcomes and independence in the home

• Train and supportcaregivers to ensure safe and effective care at home 

• Deliver a broad range of hands-on care,including wound care, infusions, catheter care, pain management, medication management, vital signs, post-operative care, etc.

• Collaborate closely with physicians and care teamsto ensure coordinated, high-quality outcomes 

What You Need: 

• Active RN license in state of residence 

• Current CPR certification 

• Valid driver’s license, current car insurance and reliable transportation 

Preferred Qualifications: 

• At least 1 year of nursing experience in home health (OASIS), med surge, ER or hospice

• We are open to train with the right skill set if you are available during the week initially
