Amcare Pro Home Health, Palliative Care & Hospice is seeking a compassionate and dependable Registered Nurse for a PRN field position serving patients throughout  Mckinney, Ana, Melissa, Blue ridge and surrounding communities.

Our Registered Nurses provide skilled, patient-centered care in the comfort of each patient’s home. This position helps patients safely manage their health conditions while supporting their independence and quality of life.

Responsibilities

• Perform skilled nursing visits in patients’ homes

• Complete Start of Care, Resumption of Care, recertification, routine, evaluation, and discharge visits as assigned

• Conduct comprehensive patient assessments

• Develop and update individualized plans of care

• Administer medications and treatments according to physician orders

• Provide wound care, IV therapy, disease management, and other skilled nursing services as ordered

• Educate patients and caregivers regarding diagnoses, medications, treatments, safety, and care plans

• Monitor patients for changes in condition and promptly communicate concerns

• Coordinate care with physicians, caregivers, therapists, and the interdisciplinary team

• Complete accurate and timely OASIS and clinical documentation

• Follow agency policies and applicable Medicare, state, and federal requirements

• Travel to patients throughout DeSoto, Duncanville, Oak Cliff, Lancaster, and surrounding areas

Qualifications

• Active and unrestricted Texas Registered Nurse license

• Current CPR or BLS certification

• At least one year of nursing experience preferred

• Home health, hospice, or palliative care experience preferred

• OASIS and WellSky/Kinnser experience is a plus

• Strong assessment, clinical decision-making, and time-management skills

• Proficiency with electronic medical record systems

• Valid Texas driver’s license

• Reliable transportation and current automobile insurance

• Ability to work independently and collaborate with the clinical team
