Job Description

An experienced and compassionate Home Health Registered Nurse is needed to provide high-quality nursing care to patients in their homes. This contract position emphasizes autonomy, clinical excellence, and meaningful patient interaction while supporting recovery and enhancing quality of life.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Field-based (patient homes) with daily travel and mileage reimbursement

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, recognize and report changes, and facilitate effective communication with care teams and physicians
• Educate patients and families about disease processes, medications, and preventive care measures
• Accurately complete documentation in Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, medical social workers, and aides for integrated care
• Participate in staff meetings, case conferences, and ongoing education
• Maintain compliance with regulatory requirements, policies, and procedural standards

Qualifications:

• Active RN license (state of practice, in good standing)
• Minimum 1 year of recent field Home Health experience
• Proficiency with Home Care Home Base (HCHB)
• OASIS experience required
• Strong clinical assessment, communication, and organizational abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to manage a flexible, independent schedule
• BSN preferred; experience managing 2532 points per week desired
• Knowledge of Medicare and home health compliance is a plus

$2,069 - $2,189/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
