Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment of patients based on physician orders/referrals.
• Complete a comprehensive evaluation using appropriate evaluation and assessment tools and procedures.
• Review patient records for current diagnosis, past medical history, precautions, and contraindications.
• Establish measurable goals and develop treatment plans to maximize patient independence and function.
• Regularly reassess the effectiveness of treatment plans and modify goals or treatments as needed.
• Communicate patient needs and progress to the treatment team, physician, and family members.
• Consult with medical providers regarding patient evaluation, treatment, and discharge.
• Recommend and assist in securing adaptive equipment for patients.
• Maintain and submit necessary documentation as required.
• Supervise Physical Therapist Assistants and Aides in accordance with state practice act.
• Attend and participate in internal staff development programs and obtain continuing education.

Requirements of the Physical Therapist

• KY or compact license
• Experience in home health, HCHB, and OASIS
