• 1+ years of experience
• Key Requirements Summary: 1. Experience: 2 years minimum required for submission (1 year specifically for this MSW position) 2. Cases: Acute Care Experience required; Discharge Planning/Care Coordination Experience required; strong preference for diverse CM experience including Med Surg, LLOS, ED 3.
