Key Responsibilities of the RN - Case Management - Travel

• Perform PRN/focused assessments of high-risk members
• Collaborate with primary care providers for comprehensive care plans
• Identify and implement cost-efficient, appropriate levels of care
• Coordinate non-medical support for compliance with treatment plans
• Manage care transitions with involved professionals
• Record clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate for privacy and confidentiality issues

Requirements of the RN - Case Management - Travel

• CO RN or Multi-State License
• Reliable transportation and car insurance
• Experience with HCHB
• Associate or bachelor's degree in nursing or related field
• Current, unrestricted state Registered Nurse license
• Case Management certification
• Ability to analyze complex medical information and make rational decisions
