Horizon Health in Mission, SD is seeking a full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. The organization is looking for a highly professional individual with strong communication skills who is committed to coordinating all aspects of primary care for the patients served. Key Responsibilities:

• Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
• Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
• Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.
