Licensed Practical Nurse (LPN)

Licensed Practical NurseWe are seeking a dedicated Licensed Practical Nurse (LPN) to support an outpatient primary care team in Ellenville. The LPN will work closely with providers and other clinical staff to provide patient care, assist with clinical procedures, obtain vital signs, administer medications as directed, and support efficient day-to-day operations of the health center.Responsibilities include obtaining and documenting patient vital signs and medical histories, preparing patients for examinations and procedures, assisting physicians, NPs, and PAs with patient care and clinical procedures, administering medications, injections, and treatments as ordered, performing routine nursing tasks, providing patients with basic health education and instructions, assisting with preventive care and chronic disease management activities, documenting patient information accurately in the electronic medical record, assisting with follow-up calls, referrals, and coordination of patient care, communicating patient needs and clinical information to the appropriate members of the care team, maintaining a clean, safe, and organized clinical environment, and providing respectful, culturally sensitive care to a diverse patient population.Qualifications include an active New York State LPN license, graduation from an accredited practical nursing program, current BLS certification, prior outpatient, primary care, community health, or FQHC experience preferred, strong communication and interpersonal skills, ability to work effectively as part of a multidisciplinary healthcare team, and bilingual skills are a plus.Why consider this opportunity?
