FT Eve: 3 pm-11:30 pm w/ every other weekend and rotating Monday & Friday (Shift Differential $1.25)Position SummaryThe Licensed Practical Nurse (LPN) provides direct nursing care under the supervision of a Registered Nurse (RN) or physician. LPN supports patient care activities, administers medications, monitors patient conditions, and assists with treatment plans while ensuring adherence to clinical protocols, safety standards, and regulatory requirements. This role promotes a safe, therapeutic, and patient-centered care environment.Essential Duties and ResponsibilitiesDeliver basic nursing care to patients in accordance with established policies and proceduresMonitor and record vital signs, symptoms, and responses to treatmentAdminister medications, injections, and treatments as ordered within scope of practiceAssist RNs or physicians with patient assessments and care planningCollect specimens and perform basic diagnostic testsProvide wound care, dressing changes, and implement infection control measuresMaintain accurate, timely documentation in electronic health records (EHR/EMR)Educate patients on medications, treatments, and self-care strategiesObserve and promptly report changes in patient condition to supervising staffMaintain a safe, clean, and therapeutic care environmentCollaborate effectively with interdisciplinary healthcare team membersAdhere to HIPAA, OSHA, and all applicable state and federal regulationsRequired QualificationsGraduate of an accredited Practical Nursing ProgramCurrent, active Licensed Practical Nurse (LPN) license in the state of practiceCurrent CPR/BLS certificationKnowledge of nursing principles, practices, and proceduresAbility to follow clinical protocols, physician orders, and regulatory standardsStrong communication, interpersonal, and teamwork skillsPreferred QualificationsPrevious experience in acute care, long-term care, behavioral health, or correctional healthcareProficiency with electronic medical record systems (EMR/EHR)IV certification (if applicable and permitted by state regulations)Organizational ExpectationsProvides a positive and professional representation of CFG Health Systems, LLC.Promotes a culture of safety for patients and employees through proper identification, reporting, documentation, and preventionMaintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practiceAdheres to infection control policies and protocols, medication administration and storage procedures, and controlled substance regulationsParticipate in ongoing quality improvement activitiesCompletes required orientation as directed by the facilityFollows facility and OSHA safety rules and procedures while on assignmentFollows facility and CFG Health Systems Occurrence ProtocolUpholds HIPAA and PREA regulationsPunctual and dependable for assigned/confirmed shiftsMaintains security clearance for the assigned facilityPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
