Job Details With a commitment to patient‑ and family‑centered care, the professional nurse uses the nursing process in ways that value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health. Develops nursing plans of care with the patient and their designated care partner(s) to effectively address individualized needs and priorities, consistent with their cultural beliefs and values. Responsibilities Interact with co‑workers, patients, their family/designated care partner(s), and other staff in alignment with Jefferson Health’s values. Integrate evidence‑based practice, clinical expertise, patient preferences and cultural beliefs to deliver optimal healthcare. Partner with patients and their family/designated care partner(s) in identifying learning needs and priorities, providing education that is useful, understandable, and affirming. Minimize risk of harm to patients and others by adhering to established nursing practices and protocols. Use information technology to communicate, manage knowledge, mitigate errors, and support ethical decision‐making. Use data to monitor care outcomes and apply improvement methods to design and test changes that continuously improve quality and safety. Function effectively within nursing and inter‑professional teams, fostering open communication, mutual respect, and shared decision‑making. Create an environment of belonging and inclusion, demonstrating problem‑solving and conflict‑resolution skills. Demonstrate commitment to learning, pursue professional growth, and act as a preceptor to appropriate staff members. Actively participate in hospital‑ and practice setting‑based committees, including safety and quality improvement. Show willingness to give and receive feedback and integrate appropriate changes into practice. Qualifications Education: Required: Specialized Diploma, Graduate of an accredited school of nursing. Preferred: Bachelor’s Degree (BSN) or completion of BSN or appropriate advanced degree within 3 years of hire. Experience: Preferred 1 year previous experience as an RN in an acute care setting. Knowledge, Skills, and Abilities: Patient‑and Family‑Centered Practice – convey respect and collaborate with patients, family, and/or designated care partner(s) in care planning. Cultural Awareness – understand people from diverse backgrounds and connect effectively. Effective Communication – apply communication concepts, tools, and techniques; use partnership language with patients and families. Patient Assessment and Care – assess ailments and injuries, implement treatment plans. Patient and Designated Care Partner Education – use child and adult education techniques to elicit learning priorities and educate on condition, treatment, medications, and next steps. Medication Administration and Prescription – apply knowledge of medications to administer and prescribe for continuing treatment outside the hospital. Discharge Planning – partner with patient/family to develop discharge plans and arrange follow‑up care. Assessment and Diagnosis – use tools and techniques for assessment and clinical judgment. Health Information Documentation – record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, and results accurately. Licenses & Certifications: BLS, AHA; RN Licensed Registered Nurse (state of practice PA or NJ). Active RN license required. Shift: Workday Day (United States).
