- Schedule: Full-time Monday-Friday + occasional weekends (as needed)
- Location: Field-based
- Comp: 75 dollars/point Who we are Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health mode. Our ultimate mission is to dramatically improve patient access to home health care. The LVN role supports that mission by delivering safe, reliable skilled care in the home within LVN scope - with excellent communication and escalation to the RN/clinical team so patients stay safe and on track. We build technology designed around the way clinicians actually work , so you can spend less time on documentation and coordination, and more time with patients.
- Smarter charting: Record or dictate visits with AI Scribe, start with prefilled assessments, extract medication lists from photos, and keep all visit information in one easy-to-use place.
- Scheduling that protects your time: Text patients, plan more efficient routes, and stay aligned on visit changes through one clear, coordinated schedule.
- Complete in-home OT evaluations and deliver skilled OT visits.
- Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
- Recommend and train on adaptive equipment and home modifications as needed.
- Coordinate with nursing and the broader care team when condition or needs change.
- Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.

- Active Texas OT license (in good standing)
- Strong clinical judgment and comfort working independently in the field
- Comfort escalating changes in condition to RN/MD and documenting rationale clearly
- Strong communication and documentation
- Reliable follow-through on orders, frequencies, and patient goals
- Current driver's license and comfort with daily travel across assigned territory
- Able to carry/transport basic therapy equipment as needed
- Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)
- Prior home health experience
- Homecare Homebase (HCHB) experience (or similar home health EMR)
- Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
- Strong organization and time management
