In this role, you will support residents and their families by helping address social, emotional, and psychosocial needs while promoting the highest possible level of well-being.If you are empathetic, proactive, and passionate about advocating for residents, we would love to meet you.### Responsibilities* Provide medically related **social services** to help residents maintain the highest practicable physical, mental, and psychosocial well-being.* Evaluate residents’ **social, emotional, and family needs** and assist in developing individualized care plans.* Provide **emotional support and counseling** to residents and families coping with illness, disability, grief, or life changes.* Assist residents and families with **healthcare decisions and care planning**.* Coordinate with the interdisciplinary team on **discharge planning**, including referrals for community services and equipment needs.* Schedule **outpatient appointments and ancillary services** such as optometry, podiatry, dental, and psychiatric visits.* Assist residents in obtaining **community resources and support services**.* Help with **personal shopping and tracking resident belongings**, including follow-up on lost or missing items.* Maintain accurate **documentation related to resident social service status**.* Build strong relationships with **facility departments and community agencies** to support resident needs.### Requirements* **Social Services Designee Certification required*** Strong communication, listening, organization, and problem-solving skills* Ability to build positive relationships with residents, families, and staff* Experience with **PointClickCare (PCC)** preferred### Physical Requirements* Frequent standing and walking* Occasional sitting, bending, kneeling, or crouching* Ability to lift up to **25 pounds**At Alhambra Post Acute, our team is dedicated to providing compassionate, high-quality care in a supportive and collaborative environment.
