The Heights Rehabilitation and Healthcare Center is looking for an experienced, energetic, and relationship-focused Admissions Coordinator to join our team!

This is a key role within our facility, helping ensure a smooth admissions process for our residents and their families while working closely with our clinical, administrative, and marketing teams.

What We’re Looking For

Our ideal candidate is an LPN or RN with previous skilled nursing/long-term care admissions experience who understands both the clinical and operational sides of the admissions process.

Requirements:

• Current Ohio LPN or RN license – required

• Previous admissions experience – required

• Skilled nursing, rehabilitation, or long-term care experience strongly preferred

• Knowledge of the referral and admissions process

• Ability to review clinical information and help determine appropriate placement

• Strong communication, organization, and follow-through

• Comfortable communicating with hospitals, referral sources, residents, and families

• Ability to work effectively in a fast-paced environment and manage changing priorities

• Positive, professional, and team-oriented attitude

What You’ll Do

As our Admissions Coordinator, you will play an important role in welcoming new residents to The Heights and helping create a positive experience from the very first interaction.

Responsibilities include:

• Coordinate and manage the facility's admissions process

• Review referrals and clinical documentation for potential admissions

• Communicate with hospital discharge planners, referral sources, residents, and families

• Work closely with nursing and facility leadership to coordinate appropriate admissions

• Maintain timely follow-up on referrals and prospective residents

• Assist with admission paperwork and ensure a smooth transition into the facility

• Build strong relationships with residents, families, and community partners

• Collaborate with the facility team to support census and occupancy goals
