This position also supports staff education, quality improvement initiatives, and regulatory compliance.Essential Job DutiesResident Evaluation & TreatmentEvaluate residents' physical therapy needs, functional abilities, and rehabilitation potential.Develop, implement, and revise individualized treatment plans based on physician orders and clinical findings.Provide evidence-based interventions to improve mobility, strength, balance, endurance, coordination, and functional independence.Establish realistic goals and monitor progress toward achievement.Educate residents and families on treatment plans, home exercise programs, fall prevention, and safety techniques.Recommend appropriate adaptive equipment and assistive devices.Interdisciplinary CollaborationCommunicate resident progress and treatment recommendations to physicians, nursing staff, families, and interdisciplinary team members.Participate in care plan meetings, case conferences, rehabilitation meetings, and discharge planning discussions.Collaborate with rehabilitation staff to maximize resident outcomes.Support restorative nursing and rehabilitation programs.Leadership & SupervisionProvide supervision and direction to Physical Therapist Assistants (PTAs), rehabilitation aides, and students in accordance with California rWe are an equal opportunity employer.
