POSITION OVERVIEW

Sharon Healthcare Woods is accepting applications for a full-time Mental Health Professional (MHP) to join our interdisciplinary behavioral health team. SHC Woods is a Specialized Mental Health Rehabilitation Facility (SMHRF) providing recovery-oriented, person-centered mental health rehabilitation services to adults living with mental illness.

Our interdisciplinary team works together to help residents achieve greater stability, independence, recovery, and successful transition into the community. Residents participate in individualized treatment and rehabilitation services, therapeutic and recovery-focused programming, activities, life-skills development, and discharge planning.

QUALIFICATIONS

Candidates must meet applicable Illinois requirements for qualification as a Mental Health Professional (MHP). Qualifying education, credentials, or experience may include applicable combinations of:

·         A bachelor’s degree in a behavioral health, social service, rehabilitation, psychology, counseling, education, or related human-services field.

·         A bachelor’s degree in another field with qualifying supervised clinical experience in a mental health setting.

·         Licensure as a Practical Nurse (LPN).

·         Approved psychiatric rehabilitation education/certification combined with qualifying education and mental-health experience.

·         Qualifying recovery-support or family-support certification.

·         Occupational Therapy Assistant credentials with qualifying mental-health experience.

·         Qualifying combinations of education and supervised clinical experience in mental health or human services.

·         Other education, credentials, experience, or grandfathered qualifications recognized under applicable Illinois requirements.

PREFERRED QUALIFICATIONS

CADC/CDAC certification or another recognized substance-use disorder credential is a plus. Experience working with individuals with co-occurring mental health and substance-use disorders is highly desirable.

·         Experience providing services to adults living with serious mental illness.

·         Knowledge of psychiatric rehabilitation and recovery-oriented services.

·         Experience facilitating therapeutic, rehabilitation, recovery, or psychoeducational groups.

·         Knowledge of substance-use education, relapse prevention, recovery support, and community resources.

·         Strong crisis-intervention and de-escalation skills.

·         Ability to work effectively as part of an interdisciplinary team.

·         Strong written, verbal, organizational, and documentation skills.

·         Commitment to treating residents with dignity, respect, professionalism, and compassion.

JOB SUMMARY

The Mental Health Professional works as a member of the interdisciplinary team to coordinate and provide services identified through each resident’s individualized treatment and recovery plan. The MHP helps residents identify personal recovery goals, develop skills, address barriers to community living, increase independence, and prepare for a successful transition to the least restrictive appropriate setting.

ESSENTIAL DUTIES AND RESPONSIBILITIES

·         Participate in and help coordinate the interdisciplinary treatment-planning process.

·         Assess and evaluate residents’ psychiatric rehabilitation and recovery-service needs.

·         Participate in the development, review, implementation, and modification of individualized treatment and recovery plans.

·         Work collaboratively with residents to identify meaningful, measurable, and person-centered goals.

·         Monitor residents’ progress toward treatment and recovery goals and document progress, barriers, interventions, responses, and changes in needs.

·         Make appropriate internal and community referrals based on identified needs.

·         Educate residents regarding community programs, benefits, recovery resources, housing, support services, and other available resources.

·         Provide individual support and counseling within the scope of the position.

·         Facilitate psychiatric rehabilitation, recovery-focused, life-skills, psychoeducational, and other assigned groups.

·         Assist residents in developing coping, problem-solving, communication, social, and independent-living skills.

·         Support residents experiencing co-occurring mental health and substance-use concerns when applicable.

·         Participate in crisis intervention, de-escalation, and behavioral support as needed.

·         Recognize and appropriately report significant changes in a resident’s symptoms, behavior, mental status, or overall condition.

·         Ensure appropriate nursing, clinical, supervisory, or physician notification when significant or adverse changes are identified.

·         Participate in discharge and transition planning and help residents prepare for community-based or more independent living.

·         Participate in interdisciplinary team meetings, clinical reviews, staff meetings, and required training.

·         Maintain accurate, timely, complete, and professional documentation.

·         Maintain resident confidentiality and comply with applicable facility policies and regulatory requirements.

·         Maintain appropriate professional boundaries and demonstrate respect for resident rights, dignity, choice, cultural differences, and individual recovery goals.

·         Perform other duties consistent with the responsibilities of the Mental Health Professional position.
