Location: Fremont, CA

Assignment Duration: This is a 13-week contract position.

Schedule: Day shift with potential weekend coverage required.

Job Description: CVHCare is seeking a qualified Physical Therapist to serve patients within designated areas. This role includes evaluating patient conditions, developing treatment plans, and providing therapeutic interventions. The therapist will be required to achieve weekly visit expectations and may need to travel outside of the assigned service area, including potential patient visits on weekends.

Responsibilities:

• Conduct comprehensive evaluations and develop individualized treatment plans for patients.
• Provide direct physical therapy interventions based on established goals.
• Document patient progress and adjust treatment plans accordingly.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
• Be available for weekend coverage as needed.

Required Experience / Certifications / Licensure:

• Current California Physical Therapist License.
• Valid BLS certification.
• Driver's license and reliable transportation.
• Compliance with health screening requirements (TB, MMR, Varicella, HepB, TDap, Flu).
• Ability to work with Covid positive patients as required.
